LOS ANGELES COUNTY MARRIAGE LICENSE APPLICATION
As a Notary Public, we can issue your Los Angeles County marriage license to you, on the spot. California is the only state where this is allowed. (Wedding must be in Los Angeles County)
Below is the application for a Los Angeles County Marriage License. Please fill it out and submit to us. You will then need to call us at 310/325-3930 for an appointment to obtain the license. Please note that when it says state of birth, enter the US state -- if not in US, enter country only. Please fill out each field.
Names: Be sure to enter your names exactly as they are on the identification you are going to present to us. Birth Certificate may be required if you have a name different from your parents. We have to use the names that appear on your identification. Also if your current last name is different from your parents please give us the last name on your birth record in the field after your current last name.
Birthplaces are the state if USA, and the country only if not USA. Be sure to give us your mother's maiden last name, as well as her first and middle names. The maiden name is her name before she was married. Please do not list step parents unless the are on your birth record. Your marriage license should match your birth record for names of parents. If a parent is unknown or your prefer to leave that field blank just insert "Unknown".
